Instructions
Add all ingredients except the heavy cream, Parmesan cheese, and cream cheese to the Crockpot.
Cover and cook on high for 3½ hours or low for 5½ hours.
Stir in the heavy cream and cheeses. Mix well and cook on low for an additional 30 minutes.
Stir before serving.
