Instructions
Slice the sausage into ¼-inch thick rounds.
Place all ingredients into the crockpot except the heavy cream and shredded cheese. Stir to combine.
Cook on high for 3 ½ hours or on low for 5 ½ hours.
Add the heavy cream and shredded cheese. Stir well, then cook for another 30 minutes on low.
Ladle into bowls and enjoy!
